Records. Every manufacturer, producer, compounder, or vendor including dispensing physicians, o f exempt prep arations must keep a record of all sales, exchanges, gifts, or other dispositions, the entries to be made at the time of delivery. This record must show the name, address, and registry number of the dealer to whom the preparation or remedy is sold, exchanged, or given, the name and quantity of the preparation or remedy, and the date upon which delivery to the purchaser or his agent or the earner is made.
A separate record must also be kept o f disposals to persons other than dealers, showing the name of the person to whom the preparation or remedy is sold, exchanged, or given, made at the tim e of delivery, his address, the name and quantity of the preparation or remedy, and the date of delivery. The requirement o f keeping a record o f sales, exchanges, or gifts to persons other than dealers w ill be deemed to be complied with, if made pursuant to prescrip tions, issued by registered physicians, calling fo r exempt narcotic preparations or remedies, if the inform ation called for in the preceding sentence is shown on the prescription, and the prescription is kept on the narcotic prescription file.
The Government does not furnish blanks fo r the keeping of these records, but they should be in the follow ing form :

Pursuant to the authority vested in the Secretary of A gri culture under section 8 of the Soil Conservation and Domes tic Allotm ent Act, payments w ill be made in connection with the effectuation o f the purposes o f section 7 a of said act fo r 1937, in accordance with the provisions of this In
sular Region Bulletin 101 fo r Alaska and such modifications or other provisions as may hereafter be made.
The 1937 Agricultural Conservation Program has been developed in accordance w ith the provisions o f sections 8, 15, and 16 of the Soil Conservation and Domestic Allotm ent Act, but the payment of any benefits pursuant to the pro visions of this announcement is contingent upon such ap propriation, if any, as the Congress of the United States may hereafter make fo r such purpose, and the amounts o f such payments w ill be fin ally determined by such appro priation and the extent of participation in the program.
The rates of payment and the soil-building allowance set forth herein are computed upon the basis of an appropria tion of $500,000,000 fo r the 1937 program fo r the Nation and 85 percent participation by farm ers. The payments calculated in accordance with the provisions of Part I of this Bulletin 101 may be increased or decreased depending upon the extent o f participation in the Insular Region, but any such variation w ill not be in excess of 10 percent.
