A.M. on the following:
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and Gay Mens Health Crisis Inc.
GMHC, located at 307 West 38th Street, New York, NY 10018, to conduct screenings and critical services, for 200 clients and enrollment for 80 clients within the five boroughs and Community Districts. The critical services will help Transgender and Nonconforming TGNC
New Yorkers with legal assistance, mental health care, and benefits advocacy. In addition, GMHC will provide linkage to medical care and facilitate gender-affirming hormone therapy through vouchers and health insurance navigation. These services are a part of an innovative and responsive care navigation program that will significantly improve the well being of transgender and gender non-conforming TGNC New Yorkers. The contract amount shall be $250,000.00. The contract term shall be from July 1, 2018 to June 30, 2019.The EPIN is 81619L0158001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and Ackerman Institute for the Family, located at 936 Broadway, 2nd Floor, New York, NY 10010. The proposed contract is to support a range of services, to help empower the transgender and gender non-conforming TGNC community, including education programs, employment services, workforce development, and health care navigation. The contract amount shall be $200,000.00. The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0154001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and New York Immigration Coalition, located at 131 West 33rd Street, New York, NY 10001. The proposed contract is to conduct outreach and education efforts regarding healthcare access and coverage, including issues pertaining to Medicare, Medicaid and the Pregnant Women/Prenatal Care Assistance Program PCAP. The contract amount shall be $190,624.00. The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0185001.

The proposed contract to improve access to health care, addressing cultural and language barriers, and targeting resources and interventions. The contract amount shall be $195, 312.00. The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0209001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and Bedford Stuyvesant Family Health Center, Inc., 1413 Fulton Street, Brooklyn, NY 11216. The proposed contract is to conduct outreach and education efforts regarding healthcare access and coverage, including issues pertaining to Medicare, Medicaid, and The Pregnant Women/Prenatal Care Assistance Program PCAP. The contract amount shall be $137,018.00.
The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0210001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and HANAC, Inc., 49 West 45th Street 4th Floor, New York, NY 10036. The proposed contract will conduct health access outreach efforts, which will include healthcare access and coverage, including issues pertaining to Medicare, Medicaid, ADAP, and CHIP. The contract amount shall be $113,581.00. The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0211001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and William F. Ryan Community Center Inc., 279 East 3rd Street, New York, NY 10009. The proposed contract will support a range of family planning services, including preconception care, prenatal care, breastfeeding counseling, and screening for sickle cell disease. The contract amount shall be $109,375.00. The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0207001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and The Young Womens Christian Association of Queens, 4207 Parsons Boulevard, Queens, NY 11355.
The proposed contract will provide preventative care workshop topics and educate the community on benefits such as SNAP, WIC, affordable healthcare and Medicaid. The contract amount shall be $104,206.00.
The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0212001.
IN THE MATTER OF the proposed contract between the Department of Health and Mental Hygiene and Federation of Protestant Welfare Agencies, Inc, located at 40 Broad Street, 5th Floor, New York, NY

The proposed contract is to conduct outreach and education efforts regarding healthcare access and coverage, including issues pertaining to Medicare, Medicaid and the Pregnant Women/Prenatal Care Assistance Program PCAP. The contract amount shall be $190,624.00. The contract term shall be from July 1, 2018 to June 30, 2019. The EPIN is 81619L0184001.
The proposed Contractors are being funded by Line Item Appropriations or Discretionary Fund, pursuant to Section 1-02e of the Procurement Policy Board Rules.
